29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle: On or about April 8, 2021, at the Hydraulic Press serial # 2526 - Employees were exposed to amputation and caught in hazards where there were no engineering control or guarding in place to prevent employees from placing their hand inside the point of operation during production including but not limited to bushing removal and installation from the tie rod.
