1910.147(c)(6)(ii) The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. At the facility located at 2310 E Main Street, Kaukauna, WI 54130; The employer did not conduct periodic inspections of energy control procedures at least annually. Maintenance employees utilized energy control procedures to deenergize and lockout CNC equipment to conduct servicing and maintenance.

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: The employer did not provided one or more methods of machine guarding to protect operators in machine areas from hazards such as, but not limited to the following: At the 2310 E Main Street, Kaukauna, WI 54130 facility: a) The Mazak Quick Turn 350 Lathe (Machine # CLM351, Model QTN350, Serial #296794) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving/rotating parts of the tool turret or the rotating portions of the CNC lathe's chuck. Employees were exposed to these hazards while they put their hands and/or head into the machine's cabin to conduct setup/programming, chip removal or manual polishing of metal parts inside the machine on the spindle. b) The Mazak Vertical Center (Machine #FJV3580D, Model FJV-35/80, Serial #277699) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving portions of the machine's tool storage magazine or the moving/rotating parts of the tool turret. Employees were exposed to these hazards when they put their hands and/or head; into the machine's cabin to conduct setup/programming and chip removal; or into the tool magazine compartment to change tools. c) The Mazak Horizontal Center (Machine #NEX10800, Model HCN10800, Serial #295719) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the caught-in/ingoing nip-point hazards of the chip removal auger, and the moving/rotating parts of the tool turret. Employees were exposed to these hazards when they bodily entered into the machine's cabin to conduct setup/programming, bore measurements and chip removal. d) The Mazak CLM 400A Lathe (Machine # CLM400A, Model QTN400, Serial # 263966) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving parts of the tool turret, the rotating portions of the CNC lathe's chuck. Employees were exposed to these hazards when they put their hands and/or head into the machine's cabin to conduct setup/programming, chip removal or manual polishing of metal parts inside the machine on the spindle. e) The Mazak Quick Turn 500 Lathe (Machine # CLM500A, Model Nexus 500-II, Serial # 263962) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving parts of the tool turret, the rotating portions of the CNC lathe's chuck. Employees were exposed to these hazards when they put their hands and/or head into the machine's cabin to conduct setup/programming, chip removal or manual polishing of metal parts inside the machine on the spindle. f) The Mazak Vertical Center (Machine # NEX57B, Model VCN 570C, Serial #312891) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving/rotating parts of the tool turret or the caught-in/ingoing nip-points of the chip removal auger. Employees were exposed to these hazards when they put their hands and/or head into the machine's cabin to conduct setup/programming and chip removal. g) The Mazak Vertical Center (Machine # FJV3580E, Model FJV-35/80, Serial #277794) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving/rotating parts of the tool turret or the moving portions of the machine's tool storage magazine. Employees were exposed to these hazards when they put their hands and/or head; into the machine's cabin to conduct setup/programming and chip removal; or into the tool magazine compartment to change tools. h) The Shibaura Horizontal Machining Center "Boring Bar" (Machine #CBB11B, Model BP-110.P20, Serial #130227) was not provided with guarding to protect employees from the struck-by/caught-between hazards presented by the movement of the tool turret's carriage, or from the rotating parts of the tool turret during production. At the 1602 Badger Road, Kaukauna, WI 54130 facility: i) The Mazak Horizontal Center (Machine # NEX50B, Model Nexus 5000-III, Serial #269214) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving/rotating parts of the tool turret or moving portions of the machine's tool storage magazine. Employees were exposed to these hazards when they put their hands and/or head; into the machine's cabin to conduct setup/programming and chip removal; or into the tool magazine compartment to change tools. j) The Mazak Vertical Center (Machine # FJV3560A, Model FJV-35/60II, Serial #220136) was not provided with locking or interlocking doors to protect Machinist employees from coming into contact with the moving/rotating parts of the tool turret, the caught-in/ingoing nip-points of the chip removal auger or the moving portions of the machine's tool storage magazine. Employees were exposed to these hazards when they put their hands and/or head; into the machine's main cabin to conduct setup/programming and chip removal; or into the tool magazine compartment to change tools.
