29 CFR  1904.39(a)(2):The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours.  1111 Southampton Rd, Westfield, MA: On or about April 27, 2021, the employer had not reported the in-patient hospitalization of an employee within 24 hours of a work related incident.   Incident date was April 24, 2021, date of hospitalization was April 24,2021.  Agency notified on April 27, 2021.
