29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, and general slip or trip hazards.   a) Express Lane 10 Minute Oil Change Inc. on or about May 6, 2021 at the address of  2024 Tiffin Ave. Findlay, OH: in the quick lube service area employer did not ensure walking-working surfaces were maintained free from slip hazards in that the floor and side rails of the service pits (1 and 2) had oil accumulations on them creating slick walking and working surfaces for employees performing oil change activities.

Employees were exposed to fall hazards of approximately 6 feet when performing work such as guiding vehicles into the service bays, cleaning, and putting away supplies.

29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) Express Lane 10 Minute Oil Change Inc at 2024 Tiffin Ave. Findlay, OH : On or about May 6, 2021 the employer did not ensure that sufficient access or working space was provided in that there were one fifty-five gallon oil drum and one fifty-five gallon drum of coolant  directly in front of the electrical access panel in the office located at the east side of the building, preventing employees from shutting down the source of power in the event of an accident or emergency.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Express Lane 10 Minute Oil Change Inc. on or about May 6, 2021 at the address of  2024 Tiffin Ave. Findlay, OH:  a) the door of the electrical access panel located in the office was left open;  b) the Honeywell Magnetic Controller access panel in the office did not have a cover, exposing employees to electrical shock and electrocution.
