29 CFR  1910.1200(e)(1): The employer did not develop, implement, and maintain a written hazard communication program which describes product labels and other forms of warning, safety data sheets, and employee information and training.  On or about June 29, 2021, the employer at Express Lane 10 Minute Oil Change Inc. located at 2024 Tiffin Avenue in Findlay, Ohio 45840 did not develop, implement, or maintain a detailed hazard communication program for operations that include the use of products that contain hazardous chemicals including but not limited to Methanol, Sodium silicate, and 2-Butoxyethanol.

29 CFR  1910.1200(g)(8): The employer did not maintain copies of required safety data sheets for each hazardous chemical in the workplace , and did not ensure that safety data sheets were readily accessible to employees.  On or about June 29, 2021, the employer did not maintain safety data sheets for employees at the Express Lane 10 Minute Oil Change Inc. located at 2024 Tiffin Avenue Findlay, Ohio 45840. Products commonly used at the facility by employees contain hazardous chemicals including but not limited to Methanol, Sodium silicate, and 2-Butoxyethanol and are used several times throughout the day for servicing customers' cars and other vehicles.

29 CFR  1910.1200(h)(1): The Employer failed to provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard is introduced into the employee's work area.  On or about June 29, 2021, the employer did not provide employees at the Express Lane 10 Minute Oil Change Inc. located at 2024 Tiffin Avenue in Findlay, Ohio 45840 with effective information and initial training and retraining on hazardous chemicals they use in their work area when servicing cars and other vehicles including but not limited to the chemicals of Methanol, Sodium silicate, and 2-Butoxyethanol.
