29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 5/18/2021 employees at the Center Street roofing project were exposed to approximately 18' falls while engaged in roofing activities without fall protection.

29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grab rail, was not provided to assist the employees in mounting the ladder:  On or about 5/18/2021 the employer did not provide employees at the Center Street roofing project  a ladder capable of extending three feet above the landing.  The ladder was not secured and no grasping device provided.
