29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about May 24, 2020, the company did not establish a program to protect employees involved with servicing and maintenance of machines and equipment from the unexpected energizing, start up or release of stored energy.  Energy sources included, but are not limited to, electrical, pneumatic, and hydraulic.  The company maintains and services equipment such as repair, trouble shooting, and regular maintenance on but not limited to the main slitter line.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about May 24, 2021, the employer did not ensure one or methods of machine guarding was provided to protect the operator and helper from the point of operation of the slitter coils and shear, ingoing nip points and rotating parts of the material moving through the line, scrap choppers, and re-coiler of the slitter line.
