29 CFR  1910.184(e)(3)(iii): Thorough inspection(s) of alloy steel chain slings were not performed by a competent person and/or did not include inspections for wear, defective welds, deformation, and/or increase in length:   (a) At the pit area; On or about 18 May 2021, employees were exposed to struck by hazard in that the employer failed to ensure the alloy steel chain slings were inspected by a competent person prior to employees utilizing  chain slings that were missing or had damaged safety latches with the gantry overhead crane.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) At the Accupress Press Brake in the fabrication department; On or about 18 May 2021 , employees working and walking near the rear of the press, opposite of the controls pedestal were exposed to amputation hazards in that the employer failed to guard the press at the rear at the point of operation.

29 CFR  1910.252(b)(2)(iii):Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used.  (a) At the wiring installation area, on or about 18 May 2021: Employees walking or working adjacent to welding operations, were not provided a means of protection from arc flash ultraviolet radiation, exposing employees to eye hazards.  Fouts Brothers, Inc. DBA Smyrna Truck &Cargo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.252(b)(2)(iii), which was contained in OSHA inspection number 1458658, citation number 1, item number 1 and was affirmed as a final order on June, 1, 2021, with respect to a workplace located at 138 Roberson Mill Road, Milledgeville GA.
