29 CFR  1926.453(b)(2)(v): A body belt and lanyard was not attached to the boom or basket when working from an aerial lift.  a) Jobsite: 1403 S Columbus Blvd, Philadelphia, PA - On or about 5/27/2021, the employer did not ensure that employees working from a platform of a JLG Telescopic Boom Lift were utilizing a body belt and lanyard attached to the basket, exposing employees to fall hazards of approximately 25 feet.
