29 CFR  1926.416(a)(1):Employee(s) were permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:   (a)     For employees working working at the R.L. Timberlake Federal Building in Tampa on or about 5 May, 2021.  The employees were working on the second floor duct bank utilizing a knockout tool for a 3 inch hole. The knock out tool made contact with an energized  480 volt wire and an arc flash occurred.
