29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing roofing activities.  This was most recently observed on June 23, 2021 at 916 West 1st St, Grand Island, Nebraska when employees were working on the roof of a three story building and were not using fall protection.  The eave height was 13 feet above the ground.

29 CFR  1926.1053(b)(6):Unsecured ladder(s) were used on surfaces that were not stable and level:  The employer is failing to protect employees from fall hazards while using ladders during roofing activities.  This was most recently observed on June 23, 2021 at 916 West 1st St, Grand Island, Nebraska when employees were using a ladder that was positioned on the slope of the first story roof.  The roof pitch at this location was 8 in 12 and the eave height was 13 feet above the ground.  The ladder was not secured and its base was not stable and level.
