In the kitchen area: A rice cooker and microwave were plugged into an extension cord that was connected to another extension cord "daisy chained" which was powered from a wall outlet. Employees were exposed to electrical and fire hazards.  b. In the kitchen area: The refrigerator was plugged into an extension cord which was powered from a wall outlet. Employees were exposed to electrical and fire hazards.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   Employees who worked with chemicals in the facility were not trained on the hazards associated with the chemicals, exposing the employees to health hazards.
