29 CFR  1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed not allowing the components to fit together to maintain the scaffold's structural integrity.    Top of the scaffolding:   A railing that did not fit together with the other components was tied to the top of the scaffolding rails and was used to stand on to reach the top of the window being removed. Employees were exposed to fall hazards.

29 CFR  1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  Front of the building: A device tied to the top of the scaffolding was used to increase the working level height of the employee so they could reach the top of the windows to being removed. Employee was exposed to fall hazards.
