29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  West side of home, 182 Medford Lakes Rd, Medford Lakes, NJ:  Two employees without any fall protection were exposed to an approximate 11-12 foot fall while installing trim around the door between the deck and the interior of the home, on or about 07/16/2021.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.1053(b)(15):Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  a)  West side of home, 182 Medford Lakes Rd, Medford Lakes, NJ:  Employee climbed the first 4-5 rungs of the damaged 20 foot, aluminum extension ladder to hand up pieces of wood trim, cut to length, for installation around the exterior of the door between the deck and the interior of the home, on or before 07/16/2021.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a)	West side of home, 182 Medford Lakes Rd, Medford Lakes, NJ:  The damaged and illegibly labeled, 20 foot aluminum extension ladder, used by an employee to hand pieces of wood trim up to employees on the second floor deck, had not been tagged with "Do Not Use" or some other similar notation, on or before 07/16/2021.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
