The employer did not ensure that employees were provided and used fall protection, on or about July 19, 2021.  (b) 2025 Valley Road Shed's Unlimited Production work floor -Employees worked on a platform on a Crown order picker that was 17 feet 8 1/2 inches long by 21 inches wide to install shingles and siding on a shed.  The employer did not ensure that employees were provided and used fall protection, on or about July 19, 2021.  (c) 2025 Valley Road Shed's Unlimited Production work floor -Employees walk on a mezzanine  7 feet 6 inches above the ground to retrieve shingles and siding for sheds.
