29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  a)  North side, northwest corner, 12 Eddy Way, Evesham, NJ:  Employee, without any fall protection was working from the makeshift work platform while installing fire blocking for the exterior wall of the new multi-family home under construction, supported by the forks on the Model VR-1044 C Ingersoll Rand all terrain fork lift approximately 12' above ground level, on or about 07/22/2021.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
