29 CFR 1926.20(b)(2): The employer did not ensure that such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer.  On or about July 26, 2021, Huffman & Company as the Controlling Employer did not ensure that:  a) That employees of its subcontractor were prohibited from working from an unapproved work platform.  Employees were potentially exposed to falls of approximately 25 feet to the ground below; and  b)  Employees of Huffman & Company such as but not limited to its site superintendent were exposed to being struck by falling debris from an unapproved wooden box or being struck by collapsing components of that unapproved wooden work platform.
