29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On or about July 29, 2021, and at times prior thereto, employees were operating powered industrial trucks without the employer providing training to safely operate the equipment, exposing employees to hazards such as, but not limited to being struck by a powered industrial truck.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about June 24, 2021,  an employee suffered an injury that resulted in hospitalization. The hospitalization was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration.
