An employee performing tree removal operations using a Bobcat MT52 Mini Track Loader, PIN 523611280 with fork attachment, was operating on a slope with forks raised overhead and over the maximum weight capacity when the MT52 tipped forward crushing the victim. Instances of failure include, but not limited to:   a. Operator driving MT52 with the forks raised while driving downhill.  b. Operator driving MT52 over the maximum weight capacity of 520 pounds.
