29 CFR  1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  On or about August 11, 2021, the employer did not ensure that storage racks were level, stable and secure. The employer did not bolt the storage racks to the floor to prevent sliding or collapse.

29 CFR  1910.178(q)(1):Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.  On or about August 11, 2021 the employer did not remove two power-operated industrial trucks (Yale model 0S030E and Hyster model R30XMS3)  that were not in safe operating conditions from service. The employer did not ensure the power-operated industrial trucks made proper connection with the picker plate attachment before operation.
