On or about August 11, 2021 the employer did not remove two power-operated industrial trucks (Yale model 0S030E and Hyster model R30XMS3)  that were not in safe operating conditions from service. The employer did not ensure the power-operated industrial trucks made proper connection with the picker plate attachment before operation.
