On or about 08/12/2021, at the job site - the employer did not get written approval prior to allowing the use of forklift attachment to lift granite slabs what weights approximately 1,000 lbs. Forklift operators were not provided with the forklift capacity, operation, and maintenance instruction plates, tags, or decals, to safely operate the forklift, exposing employees to struck-by and caught-in hazards.

29 CFR  1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  a. On or about 08/12/2021, at the job site - employees using a 4.5 angle grinder to cut granite were exposed to struck-by hazards because the machine guard was removed to use a 6 inch blade.
