29 CFR  1926.502(a)(2):The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.  On or about August  9, 2021 employees were exposed to a fall of approximately 9 feet while performing framing activities on a new commercial building located at 2450 Metro Health, Cleveland, Ohio 44109.  An employee was working from the top of an interior in the process of installing roof trusses and the employer did not ensure that he was properly using fall protection.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about August  9, 2021 employees were exposed to a fall of approximately 9 feet while performing framing activities on a new commercial building located at 2450 Metro Health, Cleveland, Ohio 44109.  An employee was working from the top of an interior in the process of installing roof trusses and the employer did not ensure that he was trained on how to recognize and minimize fall hazards.
