29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about August 12, 2021, and at times prior thereto, at the establishment, the employer did not establish a Lockout/Tag-out program consisting of an energy control procedures, employee training and periodic inspections in maintenance testing operations.  This exposed employees to hazards from the unexpected released of energy.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about August 12, 2021, and at times prior thereto, the employer did not ensure machines were properly guarded.  Employees operating the machines were exposed to the following:  a.  In the Metal Fabrication and Welding shop, an hydraulic press brakes was not guarded at the point-of-operation.  Employees operated the machine with either the foot pedal or the two-hand control and this exposed the employees to caught-in hazards.    b.  In the Welding shop, the guard for a shear was not properly installed to prevent hands or fingers from contacting the blade while it is in operation.  This exposed employees to caught-in hazards.   c.  In the Welding shop, the guard for a horizontal band saw was not adjusted to cover the unused portion of the blade to prevent hands or fingers from contacting the blade while it is in operation.  This exposed employees to caught-in hazards.

29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about August 12, 2021, and at times prior thereto, at the air dry machine, the employer did not ensure that a relocatable-power-tap (RPT) was being used in accordance with the listing instructions.  This exposed the employees to a fire hazard.

29 CFR  1910.304(b)(2):  Outlet devices had an ampere rating less than the load being served.  On or about August 12, 2021, or at times prior thereto, in the break room, the employer did not ensure that a microwave oven and a refrigerator were not plugged into the same outlet.  The outlet had a current rating less than the load being served and this exposed the employees to fire hazards.
