This was most recently documented on or about August 18th 2021, Elkhorn, Nebraska.  An employee was working on a residential structure where an employee was observed walking directly below a mobile scaffolding that was in an elevated position above 9 feet - There were two other employees working on the mobile scaffolding handling tools and other construction material.

The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about August 18th 2021, Elkhorn, Nebraska.  An employee was engaged in residential framing activities using a skill saw.  This employee was exposed to struck-by hazards because he was not utilizing eye protection.

Employees were using extension cords that have damaged sheathing that generate electricity to their power tools that were being used on the job site.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employer is failing to protect employees from fall hazards associated with framing operations on new residential home located at 2601 Piney Creek, Elkhorn NE 68022; An extension ladder was used for access and egress to the roof and it did not extend at least 3 feet above the surface being accessed. Also, the ladder was also not secured at its top to a rigid support.
