29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Jobsite: On or about 08/20/2021, employees engaged in rooftop solar panel activities, were not protected by a guardrail system, safety net system or a personal fall arrest system, exposing the employees to a fall hazard of approximately 25 feet.  Louth Callan Renewables, LLC was previously cited for a violation of CFR 1926.501(b)(1), which was contained in OSHA inspection number 1510228, citation number 1, item number 2-a, and was affirmed as a final order on 03/30/2021, with respect to a workplace located at 2030 Nooseneck Hill Rd. Coventry, RI 02816.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  (a) Jobsite: On or about 08/20/2021, employees were not protected from fall hazards while accessing a roof surface from a Skyjack Scissor lift,  approximately 25 feet above the lower level, with the lift positioned approximately 1 foot from the edge of the roof.  Louth Callan Renewables, LLC was previously cited for a violation of 29 CFR  1926.451(g)(1), which was contained in OSHA inspection number 1510228, citation number 1, item number 1 and was affirmed as a final order on 03/30/2021, with respect to a workplace located at 2030 Nooseneck Hill Rd. Coventry, RI 02816.
