1926.954(b)(3)(iii)(A) (a) On or about August 30, 2021, an employee was exposed to injuries resulting from falls while accessing a metal beam of a buss tie switch, under construction, from a JLG Industries, Inc., unit #1642, aerial lift with an articulating boom without the benefit of a fall restraint system being in use.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about August 30, 2021, an employee was in the process of timing a three phase electrical buss by walking along a metal beam, without the benefit of a fall protection system being in place, when he fell approximately 19 feet to the lower working surface, suffering serious injuries.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about August 30, 2021, an employee was in the process of timing a three phase electrical buss, when he stepped out of an aerial lift and attached his lanyard to the walking/working surface at foot level, allowing a free fall in excess of six feet.
