29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Premier Contractors LLC at 12831 Delaware St, Westminster, CO: On and about September 10, 2021, the exposing employer did not ensure that two employees were protected from injury, in that two employees conducting framing work on the patio roof of a residential home were not utilizing a fall protection system. This condition exposed the employees to a fall height up to approximately 15 feet.

29 CFR  1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  (a) Premier Contractors LLC at 12831 Delaware St, Westminster, CO: On and about September 10, 2021, the exposing employer did not ensure that employees were protected from injury, in that post holes and window wells ranging from 5-6 feet in depth were not covered and marked properly.  This condition exposed employees to fall hazards of approximately 6 feet.
