Employer failed to conduct a hazard assessment during tub-resurfacing, spray painting duties by an employee, resulting in a fire-flash & blast-explosion;  An employee sustained burns requiring hospitalization.

The employer did not establish an effective respiratory protection program with procedures for selecting respirators for use, medical evaluations and maintaining respirators for employees who were required to wear tight-fitting respirators when painting.
