29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location :  North side of residential home at 114 Sycamore St. Fairhaven, MA.  On or about 9/30/21 an employee was exposed to fall hazards of more than 9 feet to the ground below, where a guardrail system or,  personal fall arrest system was not is use.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  Location: Side of home @ 141 Sycamore St. Fairhaven, MA 02719   On or about 9/30/21 employee who was tearing apart a second floor porch who fell 9 feet 4 inches to the ground below was not properly trained.
