29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   At the residential construction site located at 429 Kaylea Drive in Lee's Summit, Missouri, employees were exposed to electric shock, electrocution, and other electrical hazards while using a frayed extension cord to plug in the air compressor. The extension cord had wires exposed at the end connector and was missing the insulation and guarding that protects the wires.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the residential construction site located at 429 Kaylea Drive in Lee's Summit, Missouri, employees were exposed to fall-related hazards while performing framing work at a height greater than 18 feet without the use of guardrail systems, safety net systems, or personal fall arrest system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees were exposed to a fall hazard because the ladder was not extended at least 3 feet above the upper landing surface of the building; they were using the top step as a way to get down from the rafters.
