29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At 10707 Kingsbridge Estates Dr, Creve Coeur, Mo 63141  - On or about October 21, 2021 an employee of Chavez Enterprises LLC, was exposed to the hazard of being struck by flying objects from using a pneumatic nail gun while not wearing eye protection.

Each employee who was engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  At 10707 Kingsbridge Estates Dr, Creve Coeur, MO, 63141 - On or about October 21, 2021 Employees of Chavez Enterprises LLC, were exposed to elevated fall hazards over six feet while  working on the roof of a residential structure and were not protected by a guardrail system, safety net system or personal fall arrest system.

At 10707 Kingsbridge Estates Dr, Creve Coeur, MO, 63141 - On or about October 21, 2021 Employees of Chavez Enterprises LLC were exposed to elevated fall hazards from a non self supporting ladder which was placed with the two rails supported unequally.
