29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer failed to ensure employees wore safety glasses while utilizing power tools that presented struck-by hazards to the eyes and face. Employees were using pneumatic nail guns and circular saws while framing the house. The employees were not utilizing eye protection, thereby exposing employees to struck-by hazards to the eyes.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Framers were using an extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 123 volt electrical contact hazard.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer did not maintain and effective safety and health program to ensure hazards were identified and corrected onsite while employees performed framing work.  EVT Building LLC was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.20(b)(1), which was contained in OSHA inspection number 1475141, citation number 1, item number 1 and was affirmed as a final order on June 24, 2020, with respect to a workplace located at 5203 Meadowsview Lane, lot #50, Hilliard, OH 43026.

29 CFR  1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021,  the employer failed to train employees on the hazards found on the worksite or the procedures to control or eliminate the hazards for employees conducting framing work.  EVT Building LLC was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.21(b)(2), which was contained in OSHA inspection number 1475141, citation number 1, item number 2 and was affirmed as a final order on June 24, 2020, with respect to a workplace located at 5203 Meadowsview Lane, lot #50, Hilliard, OH 43026.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were setting second story walls without any means of fall protection, thereby exposing employees to fall hazards of up to 17 feet.  b) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were working on the second story where the stairway opening was unguarded, thereby exposing employees to fall hazards of up to 19 feet 4 inches.  c) EVT Building LLC, at a jobsite located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123: On and/or preceding October 20, 2021, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were working on the first story where the stairway opening was not adequately guarded, thereby exposing employees to fall hazards of up to 9 feet 3 inches.  EVT Building LLC was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1476705, citation number 1, item number 1 and was affirmed as a final order on August 24, 2020, with respect to a workplace located at 6207 Limewood Dr, lot #5, Westerville, OH 43081.  EVT Building LLC was previously cited for a violation of this occupational safety and health standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1475141, citation number 1, item number 3 and was affirmed as a final order on June 24, 2020, with respect to a workplace located at 5203 Meadowsview Lane, lot #50, Hilliard, OH 43026.
