Employees connected their lanyards to the guardrails of the aerial lift basket, thereby being exposed to fall hazards of 30 feet.  b) On or before October 21, 2021, at a worksite located at 5300 Crosswind Drive, Columbus, OH 43228:  the employer did not ensure employees were trained on the correct use of personal fall protection systems. Employees were not trained on fall protection equipment inspection and storage, thereby being exposed to fall hazards of 30 feet.
