29 CFR  1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   Second floor: Employees building forms for the second floor beams were wearing harnesses but were not connected to an anchor point. Employees were exposed to fall hazards.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  In front of building: The ladder used to access the second level did not extend three feet over the landing, nor was it secured at the top, exposing employees to fall hazards.
