29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Walkway in front of building: The west side of the outer walkway was over 9-feet above the ground and fall protection was not provided to protect employees from falling to the lower level. Employees were exposed to fall hazards.

29 CFR  1926.25(a):During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  a. North entrance: Lumber with protruding nails were piled up near the ramp used to access the building, exposing employees to laceration hazards.  b. West side of the building: Lumber with protruding nails were scattered around on the west side of the building near the stairway employees used to access the second floor, exposing employees to laceration hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Outside the north exit: The outer insulation on the extension cord used to power a drill was pulled from the plug exposing the internal wiring insulation. Employees were exposed to electrical and fire hazards.
