Employees were tasked with emptying a dust bin/collector in to a dump truck while standing on the frame crossbracing for the dust collector that was 7 feet or more above the ground.  While standing on the crossbracing,  employees would pull open a sliding gate at the bottom of the dust bin, used a rod to jab inside the chute to dislodge dust and close the gate as needed.  No means of fall protection was provided to protect employees thereby exposing them to falls exceeding 7 feet.

A 12 inch section of unused blade was exposed during this operating cycle which exposed employees to struck-by hazards to the fingers and hands.
