29 CFR  1910.135(a)(1):The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  (a) Mendez Tree Removal & Lawn Care LLC, 1637 Uinta St, Denver, CO 80220: On 11/16/2021, the employer did not ensure that employees working on the ground below tree trimming operations were wearing proper head protection. This exposed employees to falling object and struck-by hazards.

29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Mendez Tree Removal & Lawn Care LLC, 1637 Uinta St, Denver, CO 80220: On 11/16/2021, the employer did not ensure that an employee working on in a tree utilizing a chainsaw was wearing proper eye and face protection. The employee was cutting the tree with a chainsaw while saw dust and wood chips were striking him in the face. This exposed the employee to struck-by hazards that could result in irreversible disabilities.
