29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Gonderflex 4-Color Flexographic Press (Model No: GFLX4000, Serial Number: GF 1024): On or about September 19, 2021 through February 9, 2022, operator(s) performed cleaning of printer rollers at stations #1 through #4 and were exposed to amputation/degloving hazards at ingoing nip points between the anilox and plate cylinder rollers. The employer did not develop, document and require printer operators to utilize machine-specific lockout/tagout procedures for the control of hazardous energy sources. Alternate protective measures, such as machine guarding were not provided. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Gonderflex 4-Color Flexographic Press (Model No: GFLX4000, Serial Number: GF 1024), Print Stations #1 through #4. On or about September 16, 2021, through February 9, 2022, the employer did not require the operator to deenergize and lock out the press prior to performing servicing and maintenance activities such as cleaning of rollers on the press while running. Operators were exposed to laceration and amputation hazards at unguarded ingoing nip points between the anilox and plate cylinder rollers at the following locations. Alternate protective measures such as machine guarding were not provided. a) Station #1 b) Station #2 c) Station #3 d) Station #4 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Machine operators were exposed to ingoing nip points, and unguarded rotating parts on the following machines: a. Gonderflex 4-Color Flexographic Press (Model No: GFLX4000, Serial Number: GF 1024). On or about December 1, 2021 through February 9, 2022, operators were exposed to amputation and degloving hazards when stirring paints and chemicals in paint troughs with a spatula in close proximity to the ingoing nip points between the anilox and plate cylinder rollers on Stations #1 through #4. Employees are also required to clean the rolls. Machine guarding was not present. b. Gonderflex 4-Color Flexographic Press (Model No: GFLX4000, Serial Number: GF 1024). On or about December 1, 2021, through February 9, 2022, employees were exposed to caught-in and fracture hazards from unguarding moving parts on the paper roll stands at the entrance and exit ends of the press. c. Jennerjahn (Slitter/Rewinder) #5 (Model No: JLS-73/18; S/N: 11359.1105). On or about December 1, 2021, through February 9, 2022, operators placed shafts into an open area on the machine, below the area that was protected by an intrelocked barrier guard. The opening between the bottom edge of the guard and the machine frame was too large to prevent contact with the ingoing nip point between the rollers inside the machine. Machine interlock was disabled at the opening where the employee reached into the machine. Machine guarding was not present. Abatement documentation is required for this item in accordance with the requirements of 29 CFRF 1903.19(d).
