29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about November 29, 2021, on site of a residential structure located at 19570 Tree Stand terrace, Lot 221, Wellington, Florida, an employee was exposed to fall hazards when performing  asphalt shingle installation work on the roof at  approximately 10 feet from ground level with a 25-feet  lifeline that was not rigged to prevent contact with the lower level.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 29,2021, on site of a residential structure located at , at 19570 Tree Stand terrace, Lot 221, Wellington, Florida, an  employee was exposed to a fall hazard of approximately 10 feet, when performing  installation work on the roof of a residential building prior to being provided with a training program that would enable the worker to recognize and avoid hazards associated with falls.
