29 CFR  1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) At 5959 Fairington Road, building 27, Lithonia, GA on December 13, 2021, an employee relocated an aluminum extension ladder in proximity to an overhead power line, exposing employees to a 20,000 volts electrical hazard.

29 CFR  1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:  (a) at 5959 Fairington Road, building 27, Lithonia, GA on December 13, 2021, the employer did not assess the site for overhead transmission line hazards.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  (a) at 5959 Fairington Road, building 27, Lithonia, GA, on December 13, 2021 workers were exposed to fall hazards while working on a 28-foot extension ladder, in that the employer failed to provide ladder training to recognize and minimize hazards related to selecting the appropriate ladder for the task.

29 CFR  1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections:  At 5959 Fairington Road, building 27, Lithonia, GA, on December 13, 2021, workers were exposed to fall hazards where a 12-foot extension ladder section was connected to the 28-foot extension ladder, held together with a tie-down strap to create a longer ladder.
