29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about December 13, 2021, an employee was handing materials up to another employee working from an elevated pump jack scaffold without using head protection, exposing the employee below to the hazard of being struck-by falling materials and equipment.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. On or about December 13, 2021, employees were working from a Werner pump jack that was not anchored at the top or bottom of the poles from heights up to approximately 13 feet, exposing employees to a fall hazard.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about December 13, 2021, and at times prior thereto, employees were installing exterior siding on a residential home under construction up to approximately 13 feet high without fall protection, exposing employees to a fall hazard.
