29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: At a commercial worksite located at 7441 Old Tiffany Springs Rd, KC MO 64153 -- Employees engaged in roofing duties of a commercial apartment building were not wearing fall protection or utilizing any fall arrest system. This exposed employees to fall hazards greater than 6 feet. This was documented on or about 12/21/2021.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access: At a commercial worksite located at 7441 NW Old Tiffany Springs Rd, KC MO -- employees engaged in roofing duties using a portable extension ladder to gain access to / from the roof did not have it extended 3 feet above the landing surface. This exposed employees to fall hazards. This was documented on or about 12/21/2021.
