(a) Location - 18 Francis St, North Reading, MA:  On or about 12/21/2021, Loyalty Home Solutions LLC, as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. Employees of subcontractor, Yupanqui Construction Inc, were exposed to fall hazards of greater than 20 feet above a lower level.
