29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:   (a)	Precision Drilling Company, L.P. located 1 mile west of the intersection of County Roads 46 & 13, Milliken, CO 80543: On and before January 8, 2022, the employer did not ensure that the Load Hitch Cover located on the bridge floor behind the doghouse of Drill Rig #461 was properly maintained.  An employee was removing ice off the grated flooring when the walking-working surface he was standing on gave way.  The employee sustained serious injuries after he fell approximately 21 feet and struck the lower level.  This condition exposed employees to fall and struck-by hazards.
