29 CFR  1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about January 13, 2022, and times prior to, two employees working from a JLG Aerial Lift, were not protected from falling by the use of a full body harness and lanyard attached to the basket. The employees were exposed to a fall hazard of approximately more than 16-feet to the concrete below.
