29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: On or about 1/14/2022, at the jobsite on the southeast corner of the structure on the second and third level platforms of the south end: The scaffold boards extended approximately 2 feet and 6 feet beyond the frame supports and the end frames were not equipped with guarding to prevent an employee from walking out on the cantilevered end, exposing them to the hazard of falling from the scaffold.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide: On or about 1/14/2022, at the jobsite on the south side, west side and east side of the southeast corner of the structure where the three-foot wide (nominal) scaffolds were not braced or tied to the structure at 12 feet. The employees were working from the second level platform at a height of approximately 14.5 feet scaffold and were exposed to the hazard of falling from the scaffold.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: On or about 1/14/2022, at the jobsite on the south side and east side of the southeast corner of the structure, employees were observed climbing the cross braces and frames of the scaffold to access and egress from the second platform level, measured to be approximately 14.5 feet above the ground, a practice that exposed them to the hazard of falling from the scaffold to the ground.
