This exposed the employee to struck-by hazards of debris from the spoil pile. This was documented on or about 01/27/2022.

29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  At a residential worksite located at 7145 NW Forest Lakes Dr, KC MO 64152 -- Employee working in the trench did not have a ladder or other acceptable form of access / egress to / from the trench. The employee climbed up the trench wall which was sliding out from under his feet as he was exiting the trench. This exposed the employee to cave-in hazards . This was documented on or about 01/27/2022.
