29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  Worksite located at 612 W. 47th St., Kansas City, Missouri- Employee was exposed to a fall hazard while working in an aerial lift and had failed to connect his harness to a lanyard to prevent the employee from being catapulted out of the lift.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Worksite located at  612 W. 47th St., Kansas City, Missouri- Employee was exposed to overhead hazards was observed working directly beneath an employee using tools and equipment in which the employee working below had taken his hard hat off his head.   The Shawhan Sheet Metal, LLC,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a)., which was contained in OSHA inspection number 1238983,  citation number 1, item number 1, and was affirmed as a final order on June 30, 2017, with respect to a workplace located at 9860 W. 87th St., Overland Park, KS.
