29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about: February 8, 2022 Location: 20 Wilson Ave. Spring Valley, NY 10956 a- (West Side) An employee engaged in roofing activities on a 6 on 12 pitched roof, three story 9-unit building fell approximately 30ft suffering fatal injuries as a personal fall arrest harness was donned but not secured to the roof via fall protection anchorage. b-(West Side) Employees engaged in roofing activities on a 6 on 12 pitched roof, three story 9-unit building were exposed to fall hazards of approximately 30ft to the ground below as personal fall arrest harnesses were donned but not secured via fall protection anchorage.
