29 CFR 1910.37(a)(3):Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a) Hosea Project Movers, at the jobsite located at 5555 Scarborough Blvd., Columbus, Ohio 43232: On and/or preceding February 8, 2022, the access to an emergency exit door in the west corner of the central warehouse was obstructed by disassembled conveyor parts and scrap metal, thereby restricting employees from safely exiting the building in the event of an emergency.

29 CFR 1910.95(c)(1):The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment. a) Hosea Project Movers, at the jobsite located at 5555 Scarborough Blvd., Columbus, Ohio 43232: On and/or preceding February 9, 2022, the employer failed to administer a continuing, effective hearing conservation program for an employee engaged in demolition activities. An employee working as a laborer was exposed to continuous noise at 76.31% on February 9, 2022, which exceeded the action limit (AL) of 50%, an 8-hour TWA (time weighted average) sound level of 85 dBA. The employee's 8-hour TWA sound level was 88 dBA during the 377 minute sampling period. Time weighted average includes a zero increment for the 103 minutes not sampled. a) Hosea Project Movers, at the jobsite located at 5555 Scarborough Blvd., Columbus, Ohio 43232: On and/or preceding February 9, 2022, the employer failed to administer a continuing, effective hearing conservation program for an employee engaged in demolition activities. An employee working as a heavy equipment operator was exposed to continuous noise at 54.83% on February 9, 2022, which exceeded the action limit (AL) of 50%, an 8-hour TWA (time weighted average) sound level of 85 dBA. The employee's 8-hour TWA sound level was 87.4 dBA during the 375 minute sampling period. Time weighted average includes a zero increment for the 105 minutes not sampled.

29 CFR 1910.253(b)(2)(ii):Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. a) Hosea Project Movers, at the jobsite located at 5555 Scarborough Blvd., Columbus, Ohio 43232: On and/or preceding February 8, 2022, in the central warehouse, oxygen and acetylene cylinders were not secured to protect the cylinders from being knocked over, thereby exposing employees to a struck-by hazard.
